HIP AND EYE GUARANTEE

The guarantee as outlined  below will only be valid if the pup is registered under our kennel name using the prefix "Darkstar's"

Should the pup develop hereditary hip or retinal dysplasia as determined by the appropriate veterinary authorities' the buyer is eligible for a replacement pup (when available) or puppy cost refunded, provided the following conditions are met.

1. Proof of AKC registrations required showing the "Darkstar's" prefix.       

2. To determine hip dysplasia, an OFA (Orthopedic Foundation for Animals Inc.) reading/report must be provided to us.

3. To determine hereditary eye problems, a reading/report must be done by a certified veterinary ophthalmologist and same forwarded to us.

If the preceding criteria are met, in order to receive a replacement pup, one of the following stipulations must be adhered to:

1. Signed AKC registration papers returned to us.

2. Veterinary proof that the dog has been spayed or neutered (procedure done at owner's expense)

3.Dog returned to us (at owner's expense)

**THIS GUARANTEE BECOMES NULL AND VOID ONCE THE DOG IS BRED.

**THIS GUARANTEE APPLIES TO THE ORIGINAL OWNER ONLY.

**THIS GUARANTEE IS NULL AND VOID AFTER A PERIOD OF 27 MONTHS FROM

    PUPS BIRTH DATE.

I, the undersigned, have read the above regarding this guarantee and agree to abide by same. I understand that if I violate any part herein, the guarantee ceases to be valid.
